WHAT YOU CAN EXPECT

A Registered Nurse assessment to evaluate your home care needs.

A Registered Nurse on call 24 hours a day, 7 days a week to respond to your home care needs and concern.

Professional care from a State Licensed Practitioner.

All levels of professional staff with frequent visits by Registered Nurse Supervisor based on your special needs.

  • Skilled Nurses (RN and LVN)

    We teach and train our nurses to give patients and caregivers the ability to provide safe and independent care. We administer treatment until the patient or caregiver learns to administer their own treatment. We plan and coordinate health care needs with physicians and other professional caregivers in outpatient and community clinics.

  • Rehabilitation Services

    We provide physical, occupational, and speech therapy treatments designed to develop and restore muscle and joint function. We enhance safety in the home, improve self care abilities and/or develop communications skills.

  • Medical Social Work (MSW)

    We assist with referrals to community resources and provide counseling related to the impact of illness and/or long-term care.

  • Certified Home Health
    Care Aide (CNA, CHHA)

    We provide short term assistance with activities and personal care needs. Our team is also qualified to administer therapy.

    Please note: the following services are not covered under home health benefits:
